Aging in place comparisons
On cost alone, modifying a home usually beats senior living: the 2025 CareScout Cost of Care Survey puts national median assisted living at $6,200 a month ($74,400 a year) and a semi-private nursing home room at $315 a day, while a full accessibility retrofit generally starts around $10,000 per This Old House. But the comparison only holds if the need is environmental. Modifications fix a house; they do not supply meals, supervision, medication management or company, and when those are the real need, staying home costs more than the construction figure suggests once in-home care is added at a national median of $35 an hour.
Staying home vs. senior living. Run the honest version of this comparison, which has three columns, not two: the one-time modification cost, the ongoing in-home care cost, and the senior living cost. The 2025 CareScout survey puts a non-medical in-home caregiver at $35 an hour - about $80,080 a year at 44 hours a week - against assisted living at $6,200 a month. Forty hours of weekly help at home costs more than assisted living. Ten hours a week costs far less. Where your parent falls on that curve is the decision, and it's a curve that moves in one direction over time.
Full remodel vs. targeted safety upgrades. A full bathroom remodel averages $15,586 with a range of $6,456-$24,715 per This Old House. Targeted work - properly blocked grab bars, a seat, a handheld sprayer, better lighting, a comfort-height toilet - is a fraction of that and addresses most of the fall risk. The case for the full remodel is when the tub is the barrier and it has to go, when the floor plan itself doesn't work, or when you'd be tearing it out within a few years anyway. Otherwise targeted is the better buy, and it leaves money for the entry.
DIY vs. professional grab bars. This one has a clear answer and it isn't about skill. If you can locate studs, install proper blocking and hit the ADA's 250-pound structural requirement (§604.5), DIY is fine and the savings are real. If you're anchoring into tile over drywall with no idea what's behind it, hire it out. A bar that pulls out of the wall is worse than no bar, because someone was relying on it. The professional isn't selling you labor here; they're selling you the certainty that the wall can take the load.
New build vs. retrofit, and alerts vs. smart monitoring. Universal design in new construction is close to free - wider doors and blocked bathroom walls cost almost nothing at framing - while retrofitting the same features means demolition. If you're building or gut-remodeling anything, do it now. On monitoring: a medical alert with 24/7 monitored response ($20-$60 a month plus $10-$11 for fall detection, per NCOA) does one thing well - gets a human dispatched. Smart-home sensors are better at pattern detection for a family that's watching, and worse at emergency response. They solve different problems and the marketing frequently blurs that.
Choosing aging in place between the options
The judgement calls that decide the bill, before anyone quotes you.
- Compare three columns, not two Modification cost, in-home care cost, senior living cost. Leaving out in-home care is what makes staying home look artificially cheap, and it's the line item that grows.
- Ask what the actual unmet need is If it's stairs and a bathtub, construction fixes it. If it's meals, medication, wandering or loneliness, construction doesn't touch it and you're solving the wrong problem expensively.
- Do targeted safety work first, remodel later if needed You can always escalate. You can't un-spend $20,000 on a bathroom that turned out not to be the constraint.
- DIY the reversible things, hire the load-bearing ones Lever handles, rocker switches, lighting, non-slip mats, decluttering - DIY freely. Anything a person's weight will hang from, hire out unless you can genuinely verify the blocking.
