Aging in place costs

The number most families should run first is the comparison, not the quote. The 2025 CareScout Cost of Care Survey puts national median assisted living at $6,200 a month and a semi-private nursing home room at $315 a day — roughly $114,975 a year. Against that, a $20,000 bathroom-and-entry project that buys three more years at home is not an expensive project. It is also not a guarantee, and it is worth being honest with yourself that modifications extend independence, they don't create it.

Bathrooms dominate the budget. This Old House's 2026 data puts a full bathroom remodel at $6,456–$24,715 with a national average of $15,586, and a walk-in shower installation at $4,000–$20,000 with an average around $12,000. Within that, a prefabricated shower unit runs $4,000–$7,000, a midrange tiled shower $8,000–$13,000, and a high-end custom build $14,000–$20,000 or more. Labor accounted for 56% of walk-in shower project costs in their survey, which is why the identical-looking bid from a cheaper contractor is usually cutting labor hours somewhere you can't see.

Stair lifts price by staircase geometry, not by person. NCOA's cost guide puts a straight indoor lift at $2,500–$5,000 installed, a curved lift at $8,000–$12,000 because the track is custom-fabricated, and an outdoor lift at $4,000–$8,000. Ongoing costs are real but small: $100–$300 a year for a maintenance plan and $200–$300 for batteries every one to three years. Rentals run $175–$500 a month, which is the right call when the need is genuinely temporary — post-surgery recovery, for instance — and the wrong call after about a year.

Budget for the surprises, because they are common in this work. This Old House found that nearly a third of homeowners paid more than expected on bathroom remodels, most often for plumbing upgrades, water damage or structural repairs discovered once the walls were open. In an older home occupied by an older owner, those discoveries are more likely, not less. Hold 15–20% back before you start, and decide in advance which parts of the scope you'd drop if the contingency gets eaten.

How to choose

Price the alternative before you flinch at the quote

Write down the local monthly cost of assisted living and multiply by 12. Compare that to the one-time project cost. It reframes what "expensive" means, in both directions — if the house needs $150,000 of work, that's real information too.

Get three bids with the same written scope

Not three bids for "an accessible bathroom." Three bids for the same document, with the same fixtures, the same tile grade and the same dimensions. Otherwise you're comparing three different projects.

Ask what's behind the wall before signing

In a bathroom, the honest contractor tells you the wall might hold rot or undersized supply lines and gives you a rate for discovered work. The one who promises no surprises either doesn't know or plans to charge you later anyway.

Rent, don't buy, for temporary needs

Stair lift rental is $175–$500 a month per NCOA. That's the right structure for a recovery period. Past about a year, buying wins on math.

Spend on the bathroom and the entry before anything cosmetic

That's where falls and lockouts happen. New flooring in the living room is a nice-to-have that will look like a mistake if the shower is still a tub with a 15-inch wall to step over.

Hold a 15–20% contingency you don't tell the contractor about
