A handrail is a continuous run of graspable rail alongside a staircase, a set of steps or a passage, fixed into structure so it can take a person's weight if they slip. For someone whose balance has changed, the three things that decide whether it works are whether it runs the full length including the last step, whether the profile can actually be gripped by that person's hand, and whether there is one on both sides.

How handrail installation actually works

  1. Establish which side, and whether it needs to be both

    Where one arm or one leg is stronger, the useful rail is on the side the person can use going both up and down — which are opposite sides of the same staircase. That is the argument for two rails, and it is a bigger safety gain than any upgrade to a single one.

    Watch for: A quote to replace an existing single rail with a nicer single rail. Adding a second is usually cheaper and does considerably more.

  2. Check the grip against the actual hand

    A graspable rail is one the fingers can wrap around and hold under load. Broad decorative caps, square-edged timber and heavy mouldings look substantial and cannot be held. Have the person hold a sample before it is ordered — hand size, arthritis and grip strength all bear on it.

    Watch for: A profile chosen from a catalogue photograph. If they cannot close a hand on it, they will slide a palm along it instead, which stops a fall from being arrested.

  3. Run it the whole way, including past the last step

    Continuity is the point. A rail that ends level with the top riser or a step above the floor releases the hand exactly where the change in level occurs, and building codes ordinarily require rails to extend and return at the ends for that reason.

    Watch for: A design that stops short because a wall or a doorway is in the way. There are usually solutions — a return into the wall, a short run past the opening — and a competent installer will propose one rather than shortening the rail.

  4. Fix brackets into framing, not into finish

    Rail brackets have to land in studs, in stair stringers, in blocking, or into masonry with correctly specified anchors. A rail carries sideways shock loads that plasterboard cannot take, and the fixing is entirely hidden once the escutcheons go on.

    Watch for: Bracket spacing set to suit the studs found rather than the loads. If the studs do not line up, the fix is a mounting board behind the rail or added blocking — not fewer brackets.

  5. Deal with the ends so nothing catches

    Open ends catch sleeves, bags and cuffs. Rails should terminate in a return to the wall or the post, which is both a code expectation in most jurisdictions and a genuine practical safety feature.

    Watch for: A rail cut square and capped. It looks neat and it is a snag point at exactly the height a coat pocket travels.

  6. Treat outside steps as a separate problem

    Exterior rails deal with rain, ice, sun and salt. Materials and fixings need to suit that, anchors into concrete and masonry need to be right for the substrate, and a cold metal rail is unpleasant to grip on a winter morning — which matters, because an unused rail is not a rail.

    Watch for: Interior rail hardware used outdoors. It corrodes at the fixings, which is where you cannot see it, and the first sign is movement under load.

  7. Confirm what the code requires here, and whether a permit applies

    Rail height, graspable profile, extensions, returns and guard requirements are set by the building code as adopted locally, and they vary. Where a staircase or an exterior stair is being altered rather than just fitted with a rail, inspection may be involved.

    Watch for: A contractor who cannot say what the local requirement is. This is checkable in an afternoon at your building department, and someone doing this work for a living should know the answer.

Where that applies, a second rail is a bigger real-world improvement than almost any other stair change, and it is comparatively cheap — usually less than the cost of replacing the existing one.
It has to be a shape a hand can wrap around and hold onto under sudden load — round or gently profiled, mounted far enough off the wall that fingers pass behind it, and continuous enough that the hand never has to let go. Wide flat rails, chunky decorative mouldings and square-edged timber fail this even though they feel solid, because a palm sliding along a surface will not stop a fall. The adopted building code sets dimensional limits for this and they vary by jurisdiction, so ask your installer or your building department for the local figures rather than working from a number you read online.
No, and using one for the other is a real mistake. A handrail is a continuous run alongside a change in level, fixed into stair or wall framing, and governed by the building code. A grab bar is a short fixed handhold at a specific point of transfer, usually in a bathroom, supplied with a tested load rating and a manufacturer's installation specification. Stair rail fitted in a shower has no rating for that use, and a grab bar spanning a staircase provides no continuity. Different jobs, different products, different fixings.
Not directly, and this comes up often on plaster-and-lath walls, on walls with services running through them, and where stud spacing simply does not suit the run. The standard solution is a mounting board — a continuous timber board fixed into every stud along the flight, with the rail bracketed onto that. It is a common, entirely legitimate method, it can be painted to match, and it also lets the brackets land wherever they are needed. Reducing the number of brackets so they hit the studs that happen to be there is not a solution.

Exterior steps are shorter but they are wet, icy, unevenly lit and often unevenly built, and they are the flight that decides whether someone leaves the house at all. If a choice has to be made, ask which flight is actually stopping the person doing something they want to do.
