The main exception, per the Insurance Information Institute, is mold that results directly from a covered peril such as a burst pipe — in which case the cost of eliminating the mold may be covered, often up to a specific dollar sublimit.
The dividing line insurers use is sudden and accidental versus gradual. A pipe that bursts on Tuesday is sudden. A shower pan that has been seeping for three years is maintenance. Mold from the first may be covered; mold from the second almost never is.
Flooding is its own category. Standard homeowners policies exclude flood entirely. Federal flood insurance may cover mold or mildew damage, but only when it is directly attributable to a flood.
Read your declarations page for a mold sublimit. Many policies cap mold remediation at a fixed dollar amount even when the underlying peril is covered, and that cap can sit well below the $10,000–$30,000 that whole-house remediation costs. Some insurers sell higher mold limits as an endorsement.
Practical claim advice: document the water event immediately with photos and video, report it promptly, keep receipts for emergency drying, and do not authorize a large remediation scope before your insurer has responded. Public adjusters and remediation companies that offer to "handle the claim for you" in exchange for signing over benefits deserve careful reading before you sign anything.
